Default Gain guide for skills

I started on the Combat Mastery page, listing all of the efforts needed to raise skills. It was so I didn't have to go through the same thing I went with Combat Mastery D again:
"HEY, DOES ANYONE KNOW WHAT THE LAST UNKNOWN THING FOR COMBAT MASTERY IS?"
Yeah, I found out... when a Goblin smacked me in the face for 53 damage. <_<;

Anyway, we should go about listing requirements for leveling skills (combat, spell casting, ranger stuff), since some of them remain 'unknown,' and without any solid leads of what they could be, you could be stuck on it for a while.

Example, to get perfect training on Combat Mastery F, you have to die. At least one time.

i think you have to talk to someone then they send you out, and the thing "suddenly" appears, but im not sure,

No, you have to actually perform it/have it performed.

Example: For counterattack, you have to strike an enemy while they're in Counterattack stance.
For Defense, you have to strike an enemy in Defense, and later on, actually have to use Smash to break their Defense to gain more Defense skill.

Some of them are rather tricky to find out (E.G. having to die for Combat Skill, having to be hit by a Critical attack)
